Using the Online Data Book

The Data Book is organized into chapters, each of which represents a different data set transmitted by hospitals directly to OH-CCO.  

Each chapter is organized by Entity.  An entity typically represents a data file to be submitted to OH-CCO that contains a group of related data elements that describe the entity.  Each record represents one instance of that entity.  A record must be submitted when a new instance of that entity occurs or a change to an existing instance occurs.  For example, when a new patient is registered at an integrated cancer program, a record with the patient entity data elements must be submitted.  If a new physician is granted privileges in the hospital’s cancer program, a record with the health care professional data elements must be submitted.

The Definition describes what information the data element contains.  The Format describes how the data is to be structured.  Valid Values show what information will be accepted in this data element.  Where the number of valid values is large, an appendix will be referenced.  Purpose and Use briefly summarizes why OH-CCO needs this data element.

Searching

To find a Chapter - use the Table of Contents pane to drill-down through the chapters and topics

To find an Entity - use the Table of Contents pane to drill-down through the chapters and topics

To find a Data Element - use the Index pane, and then press CTRL+F to search the page

To search by word or phrase - use the Search box at the top of the screen, select the topic from the pop-up menu

To search within a topic - press Ctrl+F on your keyboard and enter the word or phrase
