Patient Chart Number

Patient identifier code that is unique within the healthcare facility.

Consistent with CIHI NACRS and DAD definition and format.

Uniquely identifies a patient within an RCC.  Necessary to determine a case.  Foundational to most measures and indicators.

Submitting Hospital Number

The MOHLTC healthcare facility that submits activity to OH-CCO.

CHAR 4
Numeric

Valid MOHLTC facility numbers only.  Same 3-digit number that is used for MIS submissions.  An additional leading zero is acceptable.

For current valid MOHLTC facility numbers, see Appendix A.

Allows the hospital organization submitting the data to be distinguished from the hospital site where the service was delivered.

Patient surname. Note: This should match the patient’s current HCN.

Truncate if name longer than 50 characters.  

Required for the Ontario Cancer Registry to allow linking with other data  sets (e.g. death registrations).

Patient maiden name.

Patient Gender

Gender is a one alpha character describing the sex of the patient. “Other” includes all cases, that do not fit into either M or F categories or when the patient’s sex may be unknown.

M = Male, F = Female, O = Other or Unknown

Required for the Ontario Cancer Registry to allow linking with other data sets (e.g. death registrations).  Allows sex-adjusted incidence to be calculated.

Patient Birth Date

Patient birth date.

The date the patient died. (if known)

Health Card Number

Patient's most recent health insurance number assigned by their provincial government of residence.

Alpha-numeric

The following Values are only to be used when unable to provide a Health Care Number. 

0 = insured resident of reporting province, but HCN not available

1= not applicable, includes: Out of province resident with unavailable HCN, Out of Country, no health insurance coverage from province of residence.  

To identify OHIP vs. non-OHIP patients.  Also used for registry linking, Planning and Ontario Cancer Registry.

Health Card Version

Version number of the patient’s most recent provincial health card.

As per MOHLTC OHIP rules.  Leave blank for non-OHIP patients.

Used for registry linking,Ontario Cancer Registry.

Province that issued this health card number.

Use official 2-letter abbreviation as per CIHI.  See Appendix 1.4.

Responsibility for Payment

Payment code that identifies the primary group responsible for payment of service(s) rendered.

Host Hospital Patient Chart Number

 Alternate patient identifier code that may be specific to a hospital site or location

Alpha-numeric.  Consistent with CIHI NACRS and DAD definition and format .If alpha, must be upper case.

Planning and Ontario Cancer Registry. May be used to link hospital location specific encounters to the cancer patient unique chart number.
