Data for the five Activity Entities should be selected and submitted using a full data snapshot method, where all records for applicable visit month are included in the submission.

Data should be sent in the .csv files and should include all data elements defined for the entity. Records in these entities should represent the following information:

You can send more than one .csv file for the same Activity Entity during a submission period. For example, if you have Clinic Visit data being extracted from two different systems, you can send this data as two files. However, you must ensure that there is no overlap between the files. If the same record appears in two different files for the same Activity Entity during a submission period, both records will be stored and processed, causing duplication of data.

If it is necessary to resubmit a data package containing Activity Entity files during a submission period, it is important that you click the applicable check box(es) as instructed on the data submission screen indicating that you are doing a resubmission. If you do not do this, data from both submissions will be stored and processed, causing duplication of data. (An exception to this is if you are resubmitting due to a package or file rejection error on your original submission.) 

Note also that if you submit two or more files for the same Activity Entity in the same month (i.e. two different Clinic Visit files containing different sets of data from the same Visit Month), and it is necessary to re-submit one of the files, you must re-submit the other file(s) as well. If you do not, the data that is not re-submitted will be removed from our database, and your visits will be under-reported for that month.
