For RCCs that have migrated their data from one information system to another (i.e. from Core OPIS to a host hospital system), it may be necessary to provide a mapping list of "old Chart Number to new Chart Number" each month for Patients that have been assigned a new Chart Number/MRN.
Previously, this was done on an ad-hoc basis, but in 2008/09, a new entity was added for submitting this information. This entity is not included in the ALR chapter earlier in this document, but the format is defined in the following section.
